Placement is pretty straight forward on theater systems. Left, Center, Right of TV, then the rears behind and to the outside of the back listening area.
As for using J-boxes, you can go either way, with or w/o. You can use `old work` boxes later if you decide on external speakers. I’d lean toward no boxes now as it`s easier than if you need to pry out the boxes for inwall.
Heights on speakers should be higher than lower and consider the screen height with the final decision. (Sorry, no real “right” answer for every house).
